AUSTRALIA

• AUSTRALIAN CAPITAL TERRITORY
• Download and fill in document with black biro or type. For General Purpose Applications. (CR100)
• Documentation can be mailed with proof of photo ID and payment by cheque or money order.
  

• NEW SOUTH WALES
• Read  information at 'Information Sheets' link, and then 'Applying for a National Criminal History Record Check.'
• Present the completed application form & proof of ID to a NSW Police station or NSW Police Registered Organisation.
  

• NORTHERN TERRITORY
• Download form PF166 (Including Spent Convictions.)
• All applicants are required to attend a police station and have their ID certified.
  

• QUEENSLAND
• Persons who seek a Police Certificate can make application at any police station in Queensland.
• Call the phone number on the website for further information.
  

• SOUTH AUSTRALIA
• National Police Certificate Application forms can be downloaded. Look for the download link in the centre of the page.
• You must apply in person and have original ID documents.
  

• TASMANIA
• First read the "How To Complete an Application Form. Document can be downloaded from the bottom of the page.
• Documentation can be mailed with proof of ID and payment.
  

• VICTORIA
• Look for documents to download from the right side of the Victoria Police web pge 'Our Services".
• PDF Document can be filled in online, but must be printed of and signed. It can be mailed in with with suitable ID.

• WESTERN AUSTRALIA

• Print off the PDF document National Police Certificate Application form. It is located toward the middle of the page.
• You must apply in person with suitable ID.

CANADA

SINGAPORE
